Atlantic Health System is seeking a Manager of Care Coordination for the Care Coordination Team. This exciting opportunity is located at our Corporate Offices at 465 South Street, Morristown, NJ.

Responsibilities:
Atlantic Health System is seeking a Manager of the Care Coordination department under Integrated Care.
- The qualified candidate will provide leadership, management, and operational oversight for Atlantic Health System's organization-wide care coordination team services and operations.
- The manager reports director to the Executive Director of Care Coordination.
- The manager will collaborate with clinical and operational leaders across the care continuum from the acute care setting to the ambulatory sector to ensure patients receive the right care, at the right time in the right setting. The manager will support five medical centers, Atlantic Medical Group, and two accountable care organizations.
Qualifications
Education:
- Graduate of an accredited Nursing Program
- BSN required and CCM certification preferred.
- Masters preferred
Required License:
- Current NJ Registered Professional Nursing license required. Current PA and NY Nursing license a plus.
Related Experience:
- Minimum of 5 years of management experience required and at least 7-10 years as a Registered Professional Nurse.
- Experience in implementation and ongoing operations of case management, care coordination, and transitions of care.
- Community health experience and knowledge of community healthcare resources preferred.
- Strong clinical, interpersonal, analytical, priority setting, problem solving, and organizational skills.
- Ability to work with minimum direct supervision.
- Ability to navigate EHR and databases with demonstrated ability for electronic clinical documentation.
- Clear and accurate verbal, telephonic, and written communication skills.
