Position Summary: The Supervisor is responsible for the development, implementation, and oversight of the Deaconess Hospitalist Transitions of Care Program. This individual will work collaboratively with their operational partner and will report to the Hospitalist Medical Director. The role will involve working with the Hospitalist clinical team, nursing/care management, peers, community colleagues and others to provide exemplary care across the health care continuum. The TOC program will identify opportunities to continually improve patient care and to achieve practices that exemplify MultiCare Health Systems commitment to patient centered care and community engagement.
Minimum Requirements: Five years of recent clinical hospital/ambulatory experience Experience and demonstrated skills in a supervisory or lead capacity Health industry knowledge preferred Healthcare financial and reimbursement knowledge preferred Experience with health IT system and reports desirable
Education/Licenses/Certifications: Bachelors degree in nursing or other clinically related discipline; Masters degree in a clinically related disciple is preferred Licensed registered nurse in the state of Washington
