Sinai-Grace Hospital is DMCâ��s largest hospital, offering a comprehensive heart center, cancer care, gerontology, emergency medicine, obstetrics/gynecology and cosmetic services. Under general supervision and according to established policies and procedures, acts as a case manager for community services in the Department of Psychiatry programs. Provides coordinating and linking services between various providers (i.e. Social Security Administration, Public Health, Vocational Rehabilitation, etc.). Recommends services for clients based on individual client assessments, case documents and client/family or insurance carrier preferences. Assists patients with locating housing, medical services and social services as required. Acts as a liaison between professionals and client/family and documents client progress. Assists patients to complete applications for reimbursement/assistance for services; notifies appropriate personnel when required post-discharge services are not in place. Completes billing and other documentation as well as all required paperwork in a timely manner
1. Bachelor's degree in Social Work, Psychology, Nursing or closely related field, or the equivalent combination of education and/or experience.
2. Licensure appropriate to discipline.
3. Two years of clinical experience working with mentally ill adults.
