This position functions as a Nursing Assistant at the James J. Peters VA Medical Center, Bronx, NY and participates within a patient-focused team to provide direct and collaborative health care. The NA performs a variety of technical, rehabilitative, or therapeutic procedures & activities relating to patient care in accordance with established policies & procedures under the direction and supervision of the Registered Nurse, Nurse Manager, Nursing Director, Associate Chief Nurse, or designee. The Nursing Assistant performs technical and therapeutic duties necessary for the care of patients in both the hospital and ambulatory settings. Performance of these specialized tasks is part of the overall patient treatment goals and assists the Nurses and Physicians in providing more efficient patient care services. In addition, the work involves active participation with patients' families and other members of the health care team. Primary duties include however are not limited to the following: Observes and performs personal nursing care duties and related support duties, including feeding, bathing, lifting, turning and repositioning, oral care, nail care, and related personal hygiene duties. Takes vital signs, report results to the primary Registered Nurse (RN) and records results in the medical record. Reports changes in patient condition to the RN. Weighs patients, report results to RN and enters result into the medical record. Provide age related competent care. Maintains and stores medical supply items and equipment. Places clean linen in proper storage area. Collects and bags soiled linen for the laundry. Fills water pitchers and gives ice water to patients at beginning of shift and as needed. Distributes and collects food trays. Feeds patients or assists with meals as needed. Reports to the RN and documents input and output of fluids into the medical record. Participates in patient care activities off the unit, and off-site. Accompanies patients for care and assessment at other facilities. Transports patients for procedures/tests as required. Make pertinent observations during transportation of patients to and from appropriate areas. Assists RN with a range of treatment procedures such as sitz bath, set up of equipment for oxygen therapy and reinforcing with patients to use incentive spirometer. Ambulates patients as directed by the RN. Assists RN with inventory of patient belongings and documents in the medical record.
