The Health Technician with Ambulatory Care Service will deliver patient care at the Shively, KY CBOC. Must demonstrate skill in the care of complex conditions of multiple acute and/or chronic nature and the ability to recognize adverse and/or pending dangerous situations such as changes in blood pressure, increase in blood glucose readings, etc. Acts in the capacity of liaison/expediter between patients and Primary Care Team members, as well as other practice groups, specialty services, and ancillary services. Independently establishes priorities and varies the sequence of nursing care based on the patient's condition/response to treatment in order to accomplish assigned tasks with limited supervision by the end of the tour. Performs thorough assessment of patients with complex illnesses (chronic or acute) utilizing knowledge of pathophysiology at each visit. Verifies clinical findings from assessment with professional nurse. Demonstrates knowledge of assessment parameters for specific illnesses such as hypertension and diabetes. Demonstrates knowledge off he triage concept in the prioritization of patient care. Demonstrates knowledge and skill in delivering diagnostic and therapeutic regimens to meet patient care needs such as changing dressings, delivering breathing treatments, performing EKG's, etc. Demonstrates skill and competency in emergency situations to include performance of basic cardiopulmonary resuscitation, assistance in control groups, fire, disaster, care of seizure patient, etc. Is knowledgeable of the location of emergency equipment to include crash carts in the Ambulatory Care clinics and is familiar with the location of related emergency supplies and equipment for immediate retrieval. Observes and interviews patient/significant other or family member to gather data for assessment of patient utilizing proper documentation forms such as the CDIIPI or Nursing Assessment. As assigned, demonstrates competence in the delivery and documentation of nursing care with limited supervision Reiterates to patient the information/instructions the provider has given and obtains patient's verbalization of understanding of disease/treatment from provider. Accurately performs venipuncture (phlebotomy) in order to obtain blood specimens in appropriate receptacle. Collects, accurately labels, and bags specimens such as blood, urine, or feces as ordered. Performs hematest of stool and glucose monitoring of blood. Reports finding to nurse/provider as indicated. Demonstrates knowledge of sterile technique and appropriate location of landmarks, handling of medication, and drawing up solution when performing injections. Reports abnormal results to the professional nurse. Performs portable EKG's requiring proper anatomical placement of leads on the chest wall for accurate tracing. Assists with the setting up of intravenous equipment and provides assistance to professional nurse or provider during initiation of therapy. Demonstrates skill in the proper utilization of wall and portable oxygen and promotes appropriate safety prevention. Demonstrates skill in proper set up of oxygen therapy with correct delivery of liter. Accurately performs foot examinations of diabetic patients based on guidelines using special equipment. Accurately administers immunizations/shots, i.e. influenza, pneumococcal, TB, tetanus, etc. Demonstrates knowledge of wound care when cleansing minor wounds according to instructions provided. Knowledgeable in application of simple dressings, Ace wraps, arm slings, etc. Assists with suturing minor wounds. Transcribes physicians' orders in the computer such as ordering diagnostic tests, processing admissions, sending consults, ordering labs, scheduling procedures, etc. Demonstrates competency in providing care for adults and elderly patients based upon age related needs. Consistently assures completion oft he assignment and offers important information to the individual's plan of care based on assessment findings. Completes the necessary disposition entries in the check-out functionality in VISTA for scheduled outpatient visits to ensure proper workload credit and statistical information is maintained for the medical center.
