You Will Be Responsible For:
Provides strategic leadership to transform the Well 65+ to value-based/risk medical focus by collaborating with leadership across the organization.
Manage quality incentive programs and Medicare Care Advantage Star Rating via coordination, communication, regular meetings, and reporting while also improving care gap closure and the quality of care provided to patients.
Provide support to Well 65+ Governance and operations in the form of reporting, analytics, and maintaining a current knowledge of Well 65+ processes and business decisions.
Collaborates with quality teams and Executives across the continuum and PHSO to align quality outcomes and scorecards.
Responsible for ensuring effective communication with all staff regarding policies, procedures, forms, changes, and requirements.
Assist and evaluate the submission of patient care data retrieved from EHR Systems to ensure care provided is in accordance with established CMS measures and objectives and reports applicable data to CMS/EHR incentive program through continuous education and operational support.
Assess the quality of care provided to their patients based on specific quality metrics and assist in the submission of data to determine if any reimbursement adjustments are warranted for upcoming years based on the performance of their peers.
Awareness of and proficiency with ambulatory CDI program
Awareness of and participation in development and utilization of preferred provider network
Collaborative management of all Well 65+ performance data, communications, dashboards, etc.
Participation when indicated in all payor JOC or other similar programs / meetings / committees
What You Will Need:
Bachelor’s Degree in Accounting, Business, Decision & Information Science, Finance, Healthcare Administration, or other relevant discipline, required
2 years of experience in healthcare management or healthcare data analysis, required
Master’s Degree, preferred
Healthcare operations experience, preferred
Experience with computers and EMR systems, preferred
Ability to develop and implement strategic planning and management
Demonstrates LEAN thinking
Demonstrated computer skills in MS Office Products, particularly Excel
Creativity, strong analytical skills, proven ability in problem definition and resolution, and tolerance for detail are essential attributes.
Demonstrated ability to communicate by reading, writing legibly, speaking and comprehending English effectively in order to carry out job requirements.
The Quality Outcomes Analyst will focus on strategies to improve patient outcomes and lay a foundation for Well 65+ to be successful in a health care field. The Quality Outcomes Analyst will work with the quality departments across the continuum of AdventHealth to ensure alignment for measuring outcomes. The Quality Outcomes Analyst coordinates with practices, providers, and all levels of management within Well 65+ to ensure quality measures and standards are being met and eligible providers are compliant
