The Licensed Practical Nurse provides safe, timely and individualized nursing care to patients in the Ambulatory Care setting. The incumbent interact and collaborate with patients, families, other health care and supportive personnel. Demonstrates knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Provides effective education to patients and/or family members in relation to common disease processes, medication, and/or prescribed treatment regimes. Performs complex practical nursing skills with precision and efficiency, consistent with Medical Center policy and procedure and within legal and ethical responsibilities of his/her licensure as a Licensed Practical Nurse. Demonstrates knowledge and ability to provide safe and individualized nursing care to patients with difficult, complex and rapidly changing physiological and/or emotional needs. Assists with patient education and patient groups regarding medications and disease management. Demonstrates the ability to recognize and initiate emergency measures (to include psychiatric emergencies) and demonstrates the ability to assist in resuscitation procedures during cardiac and respiratory arrest. Establishes and maintains effective interpersonal relations with others. Participates in the orientation, education and training of less experienced LPNs and shares knowledge, experience, and skills with co-workers in the form of structured in-services and 1:1 interaction. Recognizes and appropriately responds to breakage/malfunctions or loss of equipment, supply deficiencies and fire, safety or environmental hazards. Reviews view alerts throughout the day and completes by close of business daily. Documents information timely regarding patient's condition, progress and care, while adhering to current policies and procedures. Accurately documents prescribed medications/treatments according to established policies and procedures.
