This is an open continuous announcement; applicants will be reviewed and referred on a bi-weekly basis; the announcement will remain open until the position is filled. The first cut-off for consideration will be February 10, 2021. This announcement will be used to fill RN positions in Nursing-Ambulatory Care areas: Medicine Clinic, PACT, CBOCs, etc Medicine Clinic duties: The Care Manager and Specialty Care RN monitors and patient response to acute and chronic processes based on Nursing Practice Standards, including monitoring lab results and other clinical data. She/he collaborates with patients to assess and identify needs, issues, care goals, and resources in achieving desired outcomes related to diagnosis, post hospital recovery and health maintenance/wellness in developing the patient-driven holistic care plan. Common duties include, but are not limited to: Provides specialized nursing care to acute and chronically ill patients with a multitude of diagnoses. She/he uses the nursing process and evidence- based practice to collaborate with the core team (patient, specialty care provider, Primary Aligned Care Team, Clerical Associate) and families. Recognizes emergency situations and demonstrates effective action in emergencies. Utilizes patient information databases to identify at-risk Veterans requiring coordination of care, utilization of and participation in shared medical appointments, and use of decision support tools. Utilizes appropriate resources, such as, Computerized Patient Record System (CPRS) to provide efficient and effective patient care. Functions independently and demonstrates the ability to assign, organize, and delegate assessed needs to other members of the interdisciplinary team and peers. Demonstrates excellent communication skills in communicating with patient, family, co-workers and interdisciplinary team members. Ensures excellent customer service. Performs unit audits as assigned. Performs other duties as assigned by the Assistant Nurse Manager and/or Nurse Manager. Preferred Experience: Bachelor of Science Degree in Nursing (BSN) is highly desired, with Master's degree in Nursing or related outpatient clinic or specialty care services experience preferred. Approximately 4 years of nursing practice/experience. Primary Clinic Care Mgr Duties: Work Schedule: Monday thru Friday, various tours with rotating schedules; some weekends and extended hours Using a patient and family centered approach, manage and coordinate high risk patient care in collaboration with Primary Care licensed independent providers, Care Managers, specialists, and other supportive disciplines through referral. Provide disease specific intense management of illness exacerbation and progression through use of evidentiary standards of care. Complete patient visits in the hospital admitted with Ambulatory Care Conditions. Upon discharge, validate patient and family/caregiver understanding of post hospitalization instructions; address needs, and devise plan of care centered on patient. Promote veteran patient access to care through creation of supplement to licensed independent provider appointments, e.g. nurse chair visits/nurse administration visits, appointments, telephone visits, shared medical appointments, pharmacy, nutrition Knowledgeable of and utilize health improvement programs internal and external to agency; Utilize programs to improve overall health and well-being outcomes for veteran patients. Huddle daily; provide actionable information that improves access to care and meets identified veteran patient needs. Use high-risk patient identification and management tools to project impact to disease outcomes. Provide comprehensive management for these patients to prevent emergency department visits and hospital admission. Preferred Experience: Minimum of 5 years current primary care, care/case management, or direct acute nursing care experience. Recent experience using electronic medical records. Competence in use of Microsoft programs like Outlook and Excel. Demonstrated evidence-based behaviors and clinical practices that positively influence patient and team experience. Demonstrated experience in identifying and managing patient centered care of high-risk patients. Competent in critical thinking, physical and psychosocial assessments, leadership, ability to collaborate effectively with interdisciplinary as well as have effective interpersonal, verbal, and writing skills, administering injections, EKG, glucometer, collecting lab specimens, administration of vaccinations, simple dressing change, phlebotomy, patient education regarding chronic disease, tobacco use, weight monitoring, screening of patients as needed. Demonstrated strong time management skills and ability to effectively work under stressful and time sensitive conditions The incumbents of these positions will report directly to the Nurse Manager of Primary Care. This announcement will be used to fill Patient Aligned Care Team and Case Manager positions.
