This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more Functions as the Physician Team leader of their Patient Aligned Care Team (PACT) in the Community Based Outpatient Clinic (CBOC) setting at the Dubois, PA location. May perform additional duties temporary or permanent as a float provider (face-to-face or via telehealth) as those duties are assigned. Responsible for providing medical care in an outpatient setting following PACT principles. Will routinely provide medical care using Face-to-Face(F2F), VVC, and/or Telephone appointments. When performing duties as a float provider (F2F or telehealth) for remote patients, will provide care to outreach clinics as needed. Assures Veterans receive compassionate, efficient and safe patient care in their respective care line. Assures clinical care provided meets all timelines and required documentation as required by VA. Providers must review alerts daily and have no more than 200 alerts at any given time. They must encounter their notes within 72 hours. Encounter errors must be completed within 7 days. Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran. This includes daily and weekly huddles with your PACT. Attend either by skype or in person all Primary Care statewide meetings and whenever possible, participates in the clinical and teaching activities James E. Van Zandt VA Medical Center Ambulatory Care statewide meetings. Comply with all directives and policies related to space management to achieve optimal resource utilization (space, professional and administrative) to maximize clinic utilization within the PACT model. Improves quality of care by achieving: Excellence in meeting national and VISN 4 patient safety goals, Participate in PACT huddles and manage your PACT patients with diabetes, hypertension, heart disease and mental health disorders (HEDIS) with the goal to reduce your PACT metrics and obtain excellence of HEDIS metrics, Excellence in ORYX (Joint Commission) measures. Thorough knowledge of clinical and administrative procedures as they apply to inpatient and outpatient care. Knowledge of the internal organization and content of patient's medical records and the computerized patient record system (CPRS) in order to search records and extract data relevant to completeness, accuracy, and consistence with medical center requirement. Knowledge of coding and documentation requirements to maximize reimbursement. Thorough knowledge of the various forms necessary to initiate any service, test or procedure for a patient. Thorough knowledge and ability to use CPRS to document medical care, etc. Skill in handling crisis situations, effectively calming involved parties and resolving conflicts. Ability to promote good public relations with patient care and associated services for patients. Being able to provide service recovery with patient conflicts and relations with the PACT Team. Thorough working knowledge of computer terminal operation and a thorough understanding of the Vista software utilized in support of patient care and associated services for patients. Ability to learn and teach others to effectively utilize new software programs as they relate to patient care. Thorough knowledge and understanding of VA and medical center regulations, policies and procedures pertaining to eligibility, treatment, medical records and the privacy act (HIPAA). Knowledge of clinical practices and medical care terminology, in order to communicate effectively with staff and patients, and to understand the objectives, organization and procedures of a large clinical service. Knowledge of the Diagnosis Related Groups (DRGs) patient classification system, the Current Procedural Terminology (CPT) codes for identification for inpatient procedures. Work Schedule: Typical tour of duty is Monday - Friday, 8:00am - 4:30pm, though weekends and nightshifts may be required if Veteran Needs arise. Financial Discloser Report: Not required
