Position Summary: We have an exciting opportunity to join our team as ad Assistant Director-Patient Care Services-Family Health Center.
In this role, the successful candidate under the direction of and in close collaboration with the FHC Senior Director of Quality and Patient Care Services, the Assistant Director of Patient Care Services, ensures the successful implementation of the FHCs clinical core strategies and operating results by providing oversight of nursing and medical assistant services across FHC Ambulatory Clinical Sites, along with the OBGYN and Pediatric programs. The Assistant Director Patient Care Services espouses, practices and reinforces the safety culture expectations which are a top priority in the delivery of clinical services and leadership decision-making. This includes the implementation and oversight of quality, safe and efficient nursing and medical assistant care; facilitates and supports implementation of nursing and interdisciplinary care standards; fosters and demonstrates collaborative relationships across the FHC and with other disciplines; develops and sustains collaborative practice with the patient service coordinators for FHC Programs and locations, physicians and other practitioners; coordinates service-specific quality and improvement projects. The Assistant Director Patient Care Services works collaboratively with site leadership and other disciplines to ensure that nursing and medical assistant services are well planned, staffed and fully integrated to ensure a patient-centered, outcome driven approach to care.
CLINICAL Articulates an understanding of nursing process, interdisciplinary collaboration, customer service, patient/family education, and continuum of care concepts, particularly as they apply to the population served. Participates in developing, maintaining and evaluating systems that support nursing process and the delivery of safe and effective patient care. Demonstrates and enhances others understanding of nursing and patient care standards as integral and supportive of professional practice, participatory management, and values espoused by the FHC CMO office, Departments of Nursing, and the NYU health system. Demonstrates understanding of the value of professional nursing, the conceptual framework in which it is practiced, and the features of an organization that attracts and retains nurses. Initiates and sustains development of service-level nursing and interdisciplinary process standards. Encourage, promote, and facilitate a culture where all nursing staff are expected to perform and operate at the top of their professional license, therefore practicing to the full extent of their education and training, Participates in Quality meetings and workgroups, facilitating the development of processes and workflows that include nursing as critical part of the extended care delivery team. Assists in efforts to develop and implement evidence-based clinical practice. Provides resources and collaborates to design and implement processes that facilitate age- appropriate, culturally sensitive, and ethical care for patients. Establishes and maintains processes to ensure competency and appropriate utilization of staff. Monitors PSCs at sites and programs for consistency with expected meetings, events and other compliance needs.
LEADERSHIP & COORDINATION Demonstrates visibility, accessibility and effective communication that convey a strong sense of support for clinical staff and leadership. Participates in establishing clinical standards and action plans utilizing data to make strategic recommendations. Assist clinical areas with the recruitment, development and mentoring of nurses and medical assistants In collaboration with the CMO office clinical operations, holds site PSC/ nursing leadership accountable for metrics and performance targets while providing support and guidance for role advancement and professional development of formal and aspiring leadership. Works collaboratively, demonstrating partnerships with nursing staff, physician leadership, and other administrative staff to develop and maintain clinical practice standards of care. Participates in NYU and FHC committees and projects as requested. Coordinates and facilitates activities to promote service and interdisciplinary teamwork. Functions as a respectful leader of the health care team. Perform tasks necessary for the FHC as the co-chair of the Medication Safety Committee. Ensure compliance with all FHC and NYU policies and regulatory agency standards to promote and maintain quality patient care, including but not limited to The Joint Commission (TJC), OSHA/EOC, Safety/Infection Control, Quality Care Audits, and DPH licensure requirements. Will oversee FHC CLIA Laboratory Compliance, Waived Testing, Medication Management, Provision of Care, Treatment, and Services with CMO and VP for Network Integration. Will oversee program coordinator function for the Diabetes Self Management Program for the FHC. Work with Human Resources to ensure ongoing compliance with credentials necessary to maintain licensure and certification for nursing staff in the FHC, including ensuring that all site directors and PSCs have a process for tracking, reporting and 100% compliance with staff license/registration, BCLS and other necessary certification, and infection control. Coordinates all necessary meetings with site PSCs, in direct collaboration with Sr Director of Quality and Patient Care Services, VP of Operations for the FHC (or his/her designee) Organizes and ensures compliance with all device education and training for FHC Nursing staff Collates and organizes communicable disease reporting for submission by the Sr Director of Quality and Patient Care Services Assists in collecting and providing for Step II grievance hearing and drafting written responses. Track CLIA certificate expiration and assist with reapplication
EDUCATION AND DEVELOPMENT Facilitates an environment of learning that supports patient safety, and all levels of nursing practice and personnel. Fosters opportunities for staff and leadership participation in activities and educational programs that promote learning and professional development. Serves as resource to staff and leadership for development of nursing standards that are consistent with the established FHC and NYU model. Maintains clinical and leadership competency by participation in continuing education, professional organizations and other related activities. Evaluates own performance based on professional practice standards, and criteria established by accrediting/regulatory agencies. Works collaboratively with clinical and educational resources and experts to plan and implement educational offerings for staff and leadership. Implement education and training for nursing staff that enhances their ability to effectively implement the Quality initiatives. Supports academic affiliations for purposes of providing clinically rich experiences for students of nursing, and enrichment of nursing staff. Serve as FHC liaison with nursing schools, attend recruiting events, and provide speaker services in nursing schools to promote the recruitment of nurses in primary care. Develops and implements patient education programs and materials in conjunction with FHC Patient Education Committee and/or site leadership.
EVALUATION OF OUTCOMES, QUALITY IMPROVEMENT & RESEARCH Participates in and fosters staff involvement in evidence-based practice and other nursing research activities. Participates in the implementation of programs and activities to ensure compliance with regulatory agencies as pertains to nursing and /or patient care services. Facilitates development and initiation of clinical protocols and research initiatives. Participates in developing and implementing interdisciplinary outcome standards. Participates in and/or leads interdisciplinary quality and performance improvement projects pertinent to service line goals and objectives. Systematically evaluates the quality and effectiveness of nursing practice and nursing leadership programs.
-Bachelor's degree in Nursing required; Master's degree in Health Care Administration or similar preferred. -NY State RN license required -Minimum of seven to ten years experience in nursing leadership required. -Thorough knowledge of ambulatory clinical operations is required -An in-depth knowledge of Joint Commission standards and CMS conditions of participation that relates to ambulatory sites is required. -Experience with project management and program implementation. -Ability to multi-task and oversee simultaneous projects. -Possess outstanding interpersonal and communication skills. -Demonstrated management skills with the ability to guide, direct, train and interact with staff and outside contacts. -Strong conceptual and analytical skills with the ability to identify issues and opportunities and be able to solve problems. -Strong verbal and written communication skills as well as excellent listening skills. -Ability to conceptualize work flow, develop plans, train staff, and implement appropriate actions. -Ability to create a positive environment where staff are valued and respected for their contributions. -Professional demeanor. -Develops and maintains nursing requirements necessary for maintaining HRSA 330 grant funding..
