Using your critical reasoning and clinical judgement, you will assess and evaluate the needs of patients and their families and develop a plan of care for routine care, acute care and chronic disease maintenance, while providing education and counseling. In addition, you will:
Have the knowledge and skills necessary in providing care based on physical, psychological, educational, safety and related criteria appropriate to the age of patients served
Coordinate patient care with other health care team embers/departments/agencies/hospitals/managed care insurance companies
Administer medications, vaccinations and treatments as directed, and report medication errors promptly
Monitor laboratory and other study results and follows up on abnormal findings
Acts as patient advocate and offer patient/family encouragement and support
Demonstrate awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner and report findings or events to appropriate personnel
Perform efficiently in emergency/crisis situations following established protocols
Assume charge responsibility and work with minimal supervision
Participate in performance improvement activities within the ambulatory care setting
Travel for coverage as needed
Graduate of Accredited School of Nursing – Major in Nursing
A minimum of 2 years RN experience in an ambulatory care clinical setting
Current New York State RN License
Certification in Basic Cardiac Life Support
Ability to problem-solve and work with other members of the health care team
Excellent written and verbal communication skills
