Credence Management Solutions, LLC (Credence) is seeking a Patient Safety Program Coordinator (PSPC) to assist and advise the Government with the establishment and operation of the Air Force Medical Service Patient Safety Program (AFMS PSP) at the Military Treatment Facility (MTF)-level and work closely with leadership to promote a data-driven culture of safety throughout the MTF.
Identifying, recommending, and aiding in the implementation of Process Improvement (PI) leading to the formulation of policies and processes that facilitate safer care
Making recommendations for organizational changes leading to safer patient care
Staying abreast of national patient safety initiatives and their impact on the Air Force Patient Safety Program (PSP)
Administering day-to-day PSP operations encompassing all aspects of inpatient ambulatory care, and support functions
Collaborating closely with the Chief of the Medical Staff (SGH), PI staff, Quality Management (QM), and Risk Management (RM) offices
Coordinating Root Cause Analyses (RCA), Proactive Risk Assessments (PRA), and other PS initiatives brought about by event reporting and proactive risk assessment functions of the PSP
Assessing all aspects of the MTF care processes for risks of patient harm
Complying with DoD mandated patient reporting systems (TapRooT® and Patient Safety Reporting (PSR) system) and provide consultation on how MTFs should use this data to identify system and process issues that could have or did result in harm
Collaborating with credentials management and other departments to verify adherence to AFI 44-119 and Medical Quality Operations to contribute PS-related findings that assist in the assessment of the competency of AFMS personnel
Establishing a mechanism to orient and educate staff on key patient safety principles and on elements of a reliable safety culture
Assisting with facility-wide implementation of healthcare team training using techniques such as Train-the-Trainer, Train-the-Staff On-Site, and Awareness training
Conducting an appropriate event analysis (typically an RCA, All Cause Analysis, or similar methodology) on patient safety events to identify causal factors and recommend action items to correct or improve processes and systems to eliminate or decrease the risk of the event reoccurring
Assisting with planning, developing, implementing, and coordinating facility-based PSP that supports and advances AFMS Strategic Healthcare safety and reliability objectives
Serving as the AFMS PS Subject Matter Expert (SME) resource in the facility; provide advice to all levels of AF personnel to ensure proper implementation of the program.
Conducting an annual appraisal of organization-wide PS activities, training status, policies, and procedures
Collaborating items/issues related to event prevention, risk identification, risk assessment, and risk control with all facility functions
Reporting MTF patient safety events according to AFI 44-119, Section 2.7, and DoDM 6025.13 Enclosure 8, Chapter 3 instructions
Collecting, analyzing and displaying data from facility event reviews, RCAs, PRAs, and other sources
Collaborating with facility Risk Manager in managing adverse event reporting and risk assessment
Collaborating with facility Process Improvement Manager on the use of analytical tools
Assisting with the development, planning, and coordination of a comprehensive PS education program in support of AFMS Patient Safety Goals
Educating all levels of MTF staff on the PSP, their role in creating safer, more reliable care, and processes for risk identification, assessment, and control
Assist Government personnel with leading at least one healthcare FMEA/PRA every 18 months
Assist Government personnel with implementation and monitoring of the annual The Joint Commission (TJC) NPSGs within their facilities
Assisting in the administration of PI for the MTF
Bachelor’s degree, in nursing or a healthcare-related field preferred
Four (4) years of experience in healthcare or patient safety-related field required; experience in a civilian inpatient or outpatient medical facility is required
Proficiency in Microsoft tools to include Outlook, Word, Excel, and PowerPoint
Must possess ability to work, organize time, and manage multiple projects simultaneously
Attention to detail and the ability to effectively communicate patient safety concepts to leadership
Experience with patient safety aspects of healthcare accreditation (TJC) is preferable. PSPCs without such experience must become familiar with PS accreditation requirements
Must be able to work independently, organize time, and manage multiple projects simultaneously
Documented experience with performing Root Cause Analysis (RCA) and Proactive Risk Assessment (PRA); knowledge in applying analytical and evaluative methods and techniques to a broad range of functional areas in commercial quality/performance improvement practices, highly desirable
Baccalaureate level skills in written and oral communications, public speaking, and fosters positive interpersonal relationships
Attend the next available DHA Patient Safety Professional Course
Must possess ability to work collaboratively with RM and QM
