We are seeking a RN Complex Care Manager in scenic Springfield, Oregon. This position will work in the RiverBend Primary Care Clinic overseeing, and evaluating, coordination of integrated population management to improve clinical and financial outcomes. As a manager, you will develop care management programs addressing a wide scope of health care needs and conditions.
The ideal candidate is a relationship-centered nurse with strong leadership skills, patient advocacy skills, and teamwork behaviors. Your health coaching expertise and ability to manage multiple patient encounters will be utilized in applying highly technical principles, concepts, and techniques.

Qualifications for the RN Complex Care Manager include:
Minimum three years of clinical nursing experience in an ambulatory or clinic setting
Current Oregon State Registered Nurse Licensure
Valid Oregon drivers’ license
Care Management or Care Coordination experience preferred
