BHC Alhambra Hospital is a private, fully accredited, full-service acute psychiatric hospital serving children, adolescents and adults. BHC Alhambra Hospital is dedicated to providing quality behavioral health, substance abuse, and eating disorder treatment options for the communities of the San Gabriel Valley, Los Angeles County, and surrounding areas. The Assessment & Referral Counselor is responsible for facilitating facility admissions by performing initial assessment and referral activities for all in-coming and potential patients, consulting with potential patients, patient families, medical staff and other hospital staff, and counseling patients and patient families with regard to current assessments, recommended referrals and recommended required levels of care.
Key Responsibilities include:
Conducts in-person triage assessment of potential patients, and communicates that assessment to doctors and nursing staff.
Obtains and records accurate documentation for each call including clinical description, referral sources, demographic information, financial/insurance information and follow-up requirements.
Recommends appropriate referrals based on patient condition, and communicates referrals to the Assessment & Referral Director and to the appropriate facility staff.
Counsels patients and families in regard to recommended levels of care, community resources, and financial arrangements.
