Located three and a half miles from the beach in the heart of West Los Angeles, The Canyon at Santa Monica provides an exquisite healing oasis for individuals seeking recovery in an outpatient setting. We engage the whole person in the healing process and extend an invitation for each clientâ€™s loved ones to be involved in treatment to help build a solid foundation for lasting recovery.
The Intake Coordinator initiates and completes the referral and intake process, determines appropriate level of care and maintains follow-up communication with referral sources, families, patients and program staff to maintain a quality intake.
Key Responsibilities include:
Provides directions or arranges appropriate transportation services as needed to transport patients
Responsible for introducing new patients to Therapeutic Community.Â
Explains the evaluation and admission process to the potential patient/referral source and informs patient of necessary items to bring for treatment.
Documents all intake information into the TIER, MS4, Salesforce and Midas programs including demographics and all appropriate assessments.
Maintains daily census and provides assessment for appropriate intakes.
Ensures intake paperwork is complete and accurate.Â Creates intake files.Â Files intake-related documents.Â
Responsible for verifications of benefits, recording the accurate collection of co-pays, deductibles and all other out of pocket cash collections (i.e. full self-pay payment plans), at the time of admission.
