Wellstone Regional Hospital is a 100-bed acute care facility located in Jeffersonville, Indiana and has been providing quality health care to the residents of Southern Indiana and the Louisville area since 2003. Wellstone specializes in the treatment of Adolescents, Children, Adults, CD, and the Geriatric population.Â In addition to our in-patient services, we offer out-patient programs as well.Â Wellstone is seeking a dynamic and talented Dedicated Mobile Assessor, to work PRN, 3rd shift/and or 2nd shift.
Position Summary:Â The Mobile Team Assessor will be dedicated to a designated referral source and complete assessments on site in the New Albany, Indiana area.Â Must be able to work during 3rd shift hours of 11p-7a or 2nd shift, 3p-11p.
Education/Licensure: Graduate Degree in social work, mental health/behavioral sciences, or related field and/or currently licensed to practice as a Registered Nurse (RN) by the state of Indiana.Â
Experience: Minimum of two years experience in direct clinical experience in a psychiatric/mental health/chemical dependency setting.
Additional Requirements: Experience in patient assessment, referral, treatment planning and communication with external review organizations or comparable entities. Completion of CPR course and completion ofÂ CPI prior to working independently. Thorough knowledge of JCAHO,Â state and federal regulatory requirements.
