• Group is comprised of 7 physicians and 3 APCs.
• Shared call amongst all providers.
• Louisville office is the primary practice location.
• Secondary location is at Baptist Health Floyd in New Albany, Indiana (1/2 day clinic per week), as well as one week of day call at Floyd every 4-5 weeks.
• Blocked OR time at both locations
• Physicians spend 1 day in clinic
