Kingwood Pines Hospital is seeking a dynamic and talented PHP/IOP Therapist. The PHP/IOP Therapist will provide clinically indicated psychotherapy to individuals and groups. While providing the best customer service experience, the PHP/IOP Therapist provides customer service to patients, families, allied health professionals, physicians and staff from all hospital departments. Key Responsibility Include: - To maintain documentation of therapeutic services provided.
- To provide case management to assigned patients.
- To recognize signs and symptoms of chemical dependency disorders and apply/utilize appropriate treatment Iinterventions.
- To recognize signs and symptoms of mental health disorders and apply/utilize appropriate treatment interventions.
- To keep psychiatrists and treatment team informed of patients' status.
- To participate in and to document clinical supervision, if applicable.
- To attend team and therapy meetings; to provide consultation to treatment team.
- To assess all chemical dependency needs of the patient, using the twelve (12) core functions.
- To develop treatment plans.
- To provide clinical programming for patients and families.
- To complete psychosocial history.
- To interface with Utilization Review on a regular basis to determine patient treatment needs within managed care counseling.
- To complete discharge planning requirements.
